The office of Diabetes & Endocrinology of Denver has closed permanently effective
April 15, 2022.
All future appointments have been canceled.
No additional medical advice or treatment can be provided.

The practice fax number is no longer in service.
The practice phone numer is a recording only, and unmonitored.
There is no voicemail and no paging option.
The phone will be disconnected in September 2022.
All correspondence going forward should be by mail, to the office address.
Please expect lengthy response times as mail will only be picked up intermittently.
No more prescriptions can be filled by the office.
The electronic interface for refills has been inactivated.
Please inform your pharmacy of your new provider for future refills immediately if you have not already done so.
Any outstanding lab slips should now be shown to you pcp or new provider to address.
The lab will not accept lab slips ordered by Dr. Weinstein as the practice has closed.
I recommended that a copy of your records be sent to either your primary care provider or new endocrinologist. That office can then forward your records in the future if they are needed by another office for any reason.
All patients have been advised to address this repeatedly and promptly since the start of January 2022

Signed records requests can now only be mailed back to the office at this time.
As we no longer have staff to respond to these requests, the expected turn around time for records to be sent will now be lenghty.
Accessing your own records from the patient portal or having your primary care provider (or the provider that referred you to the practice) forward their records on your behalf will be faster. The office routinely sent copies of notes and labs after each visit to all providers that you had listed in your chart and requested they be included on notifications.
Once the practice records are converted into digital storage shortly, the wait time for records to be sent will increase further.
There is no charge for records to be forwarded to another provider's office.
There is a charge (per page) for patients who request the practice print out a copy of the chart and a mailing fee.
Patients may still be able to print out records on their own off the portal instead at no charge (recommended instead of paying for a copy)
while the portal is set to " read only".
This should be done by your primary care provider (or gyn if applicable).
The office can not send any referrals as we are closed.

The patient portal has been set to "read only" for the time being.
You should still be able to see documents on the portal (and print them out), but not respond to any messages or open new messages at this time.
The patient portal is no longer available for bill paying purposes. Although the link to make a one-time payment is still there, you will not be able to pay your bill online. Payment for all outstanding balances should only be mailed to the PO box on the billing statement, NOT to the office address.
All patients have been advised previously to have a primary care provider or new endocrinologist prior to office closure to ensure continuity of care .
